Submitting a Superbill

How to Submit a Superbill for Reimbursement

I do not file insurance, but I try want to make it easy for you to use your Out-of-Network (OON) benefits. If your plan allows, you may be eligible for partial reimbursement.

✅ What You Need to Submit:

  1. superbill from your therapist (this is uploaded into the client portal at the end of the month)

  2. claim form from your insurance company (download from their website)

  3. copy of your insurance card (front and back)

  4. Optional: Any additional documents your plan requires (call to verify)

Aetna

  1. Log in at: www.aetna.com

  2. Go to "Submit a Claim"

  3. Upload:

    • Completed Aetna claim form

    • Your superbill

  4. Submit and save the confirmation

Blue Cross Blue Shield of WA

  1. Go to: www.bluecrossnc.com

  2. Log in to your member portal

  3. Find "Claims & Balances" > "Submit a Claim"

  4. Upload your BCBS claim form and superbill

Cigna

  1. Log in at: www.my.cigna.com

  2. Go to "Submit a Claim"

  3. Upload your Cigna claim form and superbill

  4. Submit and track your claim

Kaiser Permanente

  1. Go to: https://wa-member.kaiserpermanente.org/home/

  2. Log in to your member portal

  3. Find "Coverage/Costs” -

  4. Claims - How to submit for claims for reimbursement and find “submit medical claim online”

Premera

TriCare

  1. Visit: www.tricare.mil and find your regional contractor:

  2. Download the TRICARE Patient’s Request for Medical Payment (DD Form 2642)

  3. Complete the form and attach:

    • Superbill

    • Copy of insurance card and any referral/authorization if required

  4. Mail your documents to the appropriate regional claims address listed on the form or the regional site

UnitedHealthcare (UHC)

  1. Log in at: www.myuhc.com

  2. Go to "Claims & Reimbursement" > "Submit a Claim"

  3. Upload your UHC claim form and superbill

❌ Medicare & Medicaid Note

  • Medicare does not reimburse clients for therapy with out-of-network or non-enrolled providers

  • Medicaid (WA) also does not allow for out-of-network superbill reimbursements
