Submitting a Superbill
How to Submit a Superbill for Reimbursement
I do not file insurance, but I try want to make it easy for you to use your Out-of-Network (OON) benefits. If your plan allows, you may be eligible for partial reimbursement.
✅ What You Need to Submit:
A superbill from your therapist (this is uploaded into the client portal at the end of the month)
A claim form from your insurance company (download from their website)
A copy of your insurance card (front and back)
Optional: Any additional documents your plan requires (call to verify)
